Fireworks Licensing Program Forms and Reports


To assist local jurisdictions, officials, clients, and the general public, all information pertaining to the Fireworks Licensing Program have been compiled into a single location. Below are all the forms for each type of license and statistical information which the licensing system provides. Fireworks Stand Licensing - Retail Sales Licenses

Fireworks Stand Permitting Requirements
Application must be received by no later than May 1 for an annual retail sales license or November 1 for New Years sales only.
