    Instructions for Completing Internet Forms

The Weekly Claims Line internet service uses an electronic form that makes use of text boxes and buttons. Below are examples of these input fields.

Text Fields:

Text boxes capture input from your keyboard. Many questions on the application will require you to fill in a text box.

Pass your mouse cursor OVER the text box. It will change to an "". When you click inside the box, a blinking "input" cursor appears and you can type in the field.

Single Line Text Box:

 
Buttons and Boxes:

You will see buttons (known as "radio buttons") and boxes on the form. The radio buttons will give you a choice between two or more items, and you will be able to select only one of them. Generally, these are questions that require a "yes-no" answer. Use your mouse to select or "click" the radio button for your response.

Example:

Did you work for any employer?     Yes No

Click the "Close" button to return to the previous page
