Entitlement Determination Forms: TAA/TRA | ATAA | Additional TRA | Other
The Employment Security Department generates the TAA / TRA / ATAA / Additional TRA Entitlement Determination, formerly known as Form 57. The completed form notifies the potential customer of eligibility for TAA / TRA / ATAA / and Additional TRA entitlement funds. The Entitlement Determination outlines whether or not the customer is entitled to funding, for each program, along with an explanation for the determination. TAA field staff, the TRA Unit, and Automated Business Systems (ABS) use information on the worksheet to provide job search, relocation, and training services to the customer.
